What do I get?

Included with purchase is the fbx model itself, some base textures for what's shown in the image, some texture variations, and a substance painter project allowing you to do what you want.

How do I install this on my avatar?

Valid question, the way i install clothing and what i recommend is to get a tool like applyaccessories (https://github.com/artieficial/ApplyAccessories/releases) and use that to apply the piece of clothing
